Vacation Bible School Liability & Medical Release

I, the undersigned parent/legal guardian, give permission for my child(ren) to participate in Vacation Bible School activities hosted by Brooker Baptist Church on June 18–20, 2026, including on-campus activities, games, transportation (if applicable), and the off-site pool party.

I understand that every reasonable precaution will be taken to ensure the safety and well-being of all participants. However, I acknowledge that participation in church activities may involve certain risks, including but not limited to accidents, injuries, illness, transportation-related incidents, and other unforeseen events.

In consideration of my child’s participation, I voluntarily assume all risks and agree to release, waive, discharge, and hold harmless Brooker Baptist Church, its pastors, staff, volunteers, drivers, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, illness, or damage that may occur during participation in VBS activities, except in cases of gross negligence or intentional misconduct.

In the event of a medical emergency, I authorize Brooker Baptist Church staff or volunteers to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am financially responsible for any medical expenses incurred.

I also grant permission for photographs or video taken during VBS activities to be used by Brooker Baptist Church for promotional, website, social media, or ministry purposes unless otherwise noted in writing.

By signing below, I acknowledge that I have read and understand this release and voluntarily agree to its terms.
